Documents for registration

Hello there, 
I'm a private entrepreneur registered in Ukraine. 
I want to verify my account to sell my products. 
I have "Extract from uniform state register of legal entities and Physical Entities - Entrepreneurs" with notary translation and stamp, but your colleagues said this document is invalid. 
Please, tell me, what document do you need to verify my account? What documents are provided by other Ukrainian companies?

@Rebellis_uaWe ask for a legible signature (name and surname) on the pep statement document. And a company registration document not older than 4 months.

Thank you for your reply! 
In Ukraine, we get the registration document when the company is open with the actual date of opening. My company opened in 2015... You can't verify this form with a date of 2015. 
I can get the new doc with a new date and translate it with the notary, but there will not be a stamp. Is this form will OK for you?
